Dear Patients:

 

Effective July 31, 2021, I, Rolando Perez, M.D., will no longer see patients at ALON Virtual Health, previously known as ALON Family Health (“ALON”). I am closing my practice. You, as my patient, are hereby given notice of the closure of my practice as required by Texas Medical Board Rule 165.5.

 

Until then, I will continue to provide necessary care limited to: prescription refill requests submitted electronically through your pharmacy; review of outstanding laboratory and imaging studies; referral authorizations; and completion of outstanding medical forms.  As these final administrative tasks are completed, please note no appointments of any type will be available. 

 

You have the right to (i) obtain copies of your medical records, or (ii) arrange to have your medical records transferred to any other physician of your choosing. If you would like to obtain a copy of your medical records or transfer them, please fax a signed release of medical records form (833) 316-1848.  The form is available at https://www.alonvirtualhealth.com/recordsrelease.

 

Until further notice, I will remain the custodian of your medical records, and all medical records requests should be directed to me at:
